Hypothyroidism

Feeling tired? Trouble losing weight? It could be your thyroid!

Hypothyroidism is a condition in which the body lacks sufficient thyroid hormone. Since the main purpose of thyroid hormone is to "run the body's metabolism," it is understandable that people with this condition will have symptoms associated with a slow metabolism. Symptoms include:

  • Fatigue
  • Weight gain or difficulty losing weight
  • Coarse, dry hair
  • Hair loss
  • Dry, rough skin
  • Cold or heat intolerance
  • Constipation
  • Depressed mood
  • Memory loss
  • Abnormal menstrual cycle
  • Decreased libido
  • Muscle cramps

The estimates vary, but approximately 10 million Americans have this common medical condition. In fact, as many as 10% of women may have some degree of thyroid hormone deficiency. Hypothyroidism is more common than you would believe, and millions of people are currently hypothyroid and don't know it.

Diagnosis of Hypothyroidism

Testing for thyroid disease is somewhat controversial. Traditional medicine relies heavily on one blood test alone, called the TSH, for diagnosis of hypothyroidism. TSH is produced by the brain and sent as a signal to the thyroid gland to produce thyroid for use in all areas of your body. The basic problem is the so called "normal range" of TSH is too high for some people. Many patients with a TSH greater than 2.0 (not 4.5, as used by traditional medicine) have classic symptoms and signs of hypothyroidism. Therefore, many patients continue to suffer needlessly.

Here at Empower Medical and Wellness,  we first listen to YOU, the patient. What are your symptoms? How are these symptoms affecting you and decreasing your quality of life? Next we look at your specific lab values which include TSH along with T4 and T3, the active hormone produced by the thyroid gland. It is not uncommon to find T4 and T3 levels below normal, even with a “normal” TSH by traditional standards. If this is the case, we move to treatment. Our goal is to treat your symptoms early, before you reach disease state by “optimizing” your thyroid hormone.

How do we treat Hypothyroidism?

The traditional approach is to prescribe Synthroid or Levoxyl (levothyroxine). Although this is helpful for some patients, it is not helpful for all patients. This is primarily because Synthroid is only T4, which still has to be converted by the body to T3. Some patients have difficulty in this conversion and are therefore unable to experience the benefit of thyroid replacement. At Empower Medical and Wellness, we also predominantly prescribe natural thyroid replacement such as armour or nature thyroid, which is a mixture of T3 and T4. This combination allows for better absorption and for many, better symptom relief. Optimizing thyroid, along with your other hormone levels such as Testosterone, Estradiol and Progesterone will allow you to look and feel your best!

Testosterone Replacement Therapy

Low energy? Lack of motivation? Decreased erections? It could be your Testosterone.

A diagnosis of low testosterone by a medical professional is the essential first step in any treatment program. If you are diagnosed as having low testosterone with or without erectile dysfunction symptoms, testosterone replacement therapy may be recommended. There are several types of delivery systems to increase testosterone levels in men, as well as treating erectile dysfunction. A consultation to discuss symptoms, lab results and treatment options by our provider is the best first step in overcoming symptoms and achieving optimal results.

What changes occur in the body due to testosterone deficiency?

  • Decrease in muscle mass, with increased body fat
  • Erectile dysfunction
  • Fatigue
  • increased joint and muscle pain

Low testosterone in men may cause a decrease or loss of sex drive, difficulty in achieving and keeping an erection, symptoms of depression, a decreased sense of well-being and difficulty concentrating.

What can cause testosterone loss?

As a man ages, the amount of testosterone in his body gradually declines. This natural decline starts after age 30 and continues throughout life. Other causes of lowered testosterone levels include:

  • Testicle loss, infection or injury
  • High Stress levels **most common factor
  • Chemotherapy
  • Genetics in family
  • Excessive iron in the body
  • Pituitary gland dysfunction
  • Inflammatory diseases
  • Certain medications
  • Some illnesses that are chronic
  • Excessive alcohol use

How do I find out if I have a testosterone deficiency?

The only accurate way to detect the condition is to have your medical provider measure the amount of testosterone in your blood. We prefer, if possible, to test levels early in the morning since this is when testosterone levels are at their highest. Here at Empower Medical and Wellness, we also look at your estradiol, vitamin D, and DHEA to see if your other hormones are fighting against your testosterone.

How is testosterone deficiency treated?

  • Intramuscular injections, generally weekly
  • Bio-Identical Pellets
  • Testosterone topical gel

Each of these options provides adequate levels of hormone replacement. However, they all have different advantages and disadvantages. We take the time to educate you on these different options and help you make an informed decision on which treatment approach may be right for you.

The only accurate way to detect the condition is to have your medical provider measure the amount of hormones in your blood. If you’re still having a menstrual cycle, we like to draw your levels on day 19 of your cycle. This means the day you started your menstrual cycle is day one and you count 19 days out. If you no longer are having a menstrual cycle, then you can have these levels pulled any day of the month. Here at Empower Medical and Wellness, we also look at your thyroid levels, vitamin D, and DHEA, and cortisol to see if your other hormones are fighting against you.

How is hormone deficiency/imbalance treated?

  • Bio-Identical Pellets (this can be estradiol or testosterone)
  • Bio-Identical topical creams (this can be estradiol, progesterone or testosterone)
  • synthetic tablets by mouth
  • Bio-Identical estradiol patches to apply to upper extremity twice a week
  • Bio-Identical capsules such as micronized progesterone and estradiol
